★「論文計畫書審查申請日期」與「學位考試日期」應間隔至少3個月以上There should be a gap of at least 3 months between the "thesis proposal review application date" and the "degree examination date"。 【請於口試辦理日期三週前備齊Please prepare it three weeks before the oral examination date.】 1.「臺灣學術倫理教育資源中心」核發之6小時以上修課證明Certificate of course attendance of more than 6 hours issued by "Taiwan Academic Ethics Education Resource Center"*1份 2. 學位論文(專業實務報告)擬聘口試委員申請表Application Form for Appointment of Oral Examination Committee Member for Dissertation (Professional Practice Report)*1份【PDF】【ODT】 3. 切結書Letter of recognizance*1份【PDF】【ODT】【EN】 4. 學位考試申請表Degree Examination Application Form*1份 (登入教務系統→碩博士學位→學位考試申請→填表列印)(學生自備) Log in to the Academic Affairs System → Master and Doctoral Degree → Application for Degree Examination → Fill in the form and print it 5. 學位論文(專業實務報告)初稿、論文(專業實務報告)初稿比對結果*各1份((學生自備) Comparison results of the first draft of thesis (professional practice report) and the first draft of thesis (professional practice report) 6. 歷年成績單正本*1份 (可至小額繳款機申請歷年成績單及繳費，將申請表連同繳費單送教務處註冊課務組)(學生自備) You can go to the small-amount payment machine to apply for past transcripts and pay fees.
